It sure sounds like it's dead.
The watering schedule should have been a heavy soaking once a week, not once a month. That's assuming it is your typical Phaleonopsis.
If there are ner leaves you might as well toss it out. If there are other leaves try soaking the whole pot in lukewarm water for a couple of hours, then drain and put in a place that has bright indirect light.
In the future you can water once a week in the sink or shower by running the water for three or four minutes.
Wait a couple of weeks and then fertilize with any orchid growth formula as directed on the label.
Cut off the stem of the flower spike that died down to the second growth node.
You can get it to rebloom next year by making sure that it gets a couple of weeks of night time temperatures fifteen degrees lower than the daytime temperature.
